Topical Therapies

Topical treatments are often the first line of defense against psoriasis. These treatments are applied directly to the skin to reduce inflammation, slow down skin cell turnover, and alleviate symptoms such as scaling and itching. Common topical therapies include:

  • Topical Corticosteroids: These anti-inflammatory medications help reduce redness and itching. They come in various strengths, and your dermatologist will prescribe the appropriate potency based on the severity and location of the psoriasis.
  • Topical Vitamin D Analogues: Calcipotriene and calcitriol help slow down skin cell growth and flatten lesions. They are often used in combination with corticosteroids for better results.
  • Topical Retinoids: Tazarotene is a vitamin A derivative that helps to reduce inflammation and slow skin cell growth. It can be used alone or in combination with other treatments.
  • Coal Tar: This traditional treatment reduces scaling, itching, and inflammation. It is available in shampoos, creams, and ointments and is particularly useful for scalp psoriasis.
  • Salicylic Acid: This helps remove scales and smooth the skin. It is often used in combination with other topical treatments to enhance their effectiveness.

Oral Therapies

For moderate to severe psoriasis, oral medications may be necessary to control symptoms and reduce inflammation:

  • Oral Retinoids: Acitretin is used to reduce the speed of skin cell growth. It is often used for severe psoriasis that does not respond to other treatments.
  • Immunosuppressants: Methotrexate, cyclosporine, and mycophenolate mofetil can be used to manage severe psoriasis by suppressing the immune system's overactivity. These medications are usually reserved for cases that do not respond to other treatments due to potential side effects.
  • Oral PDE4 Inhibitors: Apremilast (Otezla) is a newer oral medication that helps reduce inflammation by inhibiting phosphodiesterase 4 (PDE4) enzyme activity. It is effective for moderate to severe psoriasis and has a different side effect profile than traditional immunosuppressants.

Injectable Therapies

Injectable biologic therapies have revolutionized the treatment of moderate to severe psoriasis. These therapies target specific pathways in the immune system that contribute to psoriasis:

  • Tumor Necrosis Factor (TNF) Inhibitors: Medications such as adalimumab (Humira), etanercept (Enbrel), and infliximab (Remicade) block TNF, a substance involved in inflammation.
  • Interleukin Inhibitors: These include ustekinumab (Stelara), which targets IL-12 and IL-23, and newer drugs like secukinumab (Cosentyx) and ixekizumab (Taltz), which target IL-17, as well as guselkumab (Tremfya) and risankizumab (Skyrizi), which target IL-23. These medications help to reduce inflammation and skin cell production.

Phototherapy

Phototherapy, or light therapy, is another effective treatment option for psoriasis. It involves exposing the skin to controlled amounts of ultraviolet (UV) light under medical supervision:

  • Narrowband UVB Phototherapy: This is the most common form of phototherapy for psoriasis. It helps reduce inflammation and slow skin cell turnover. Treatments are typically administered two to three times a week over several months.
  • PUVA (Psoralen plus UVA) Therapy: This combines a drug called psoralen with UVA light to enhance the skin's response to light therapy. It is used for more severe cases of psoriasis but is less commonly used due to the potential for side effects.
